- Paid through date (i.e., the date through which benefits have been accrued)
- Benefit accrual start date (and/or elimination period)
- LTD Integration
- Transition to LTD (defined as receiving at least one LTD benefit payment)
- Age at date of disability
- Gender
- Primary diagnosis (preferably ICD9 or ICD10 code)
- Gross weekly benefit
